Dr. Muhammad Faisal, MD, CCFP
Dr. Muhammad Waqar Faisal is an experienced male family physician with advanced qualifications from the UK’s Royal Colleges. He is committed to staying up-to-date with the latest treatments for acute and chronic conditions and is proficient in a wide range of clinical skills to provide comprehensive care. Dr. Faisal is fluent in English, Hindi, Urdu, and Punjabi.
WHAT TO EXPECT AFTER SUBMITTING THE REGISTRATION FORM
1. Meet & Greet Appointment:
Once you complete the online form, we will schedule a meet and greet appointment with the doctor.
2. Mutual Agreement:
If both you and the doctor agree to proceed, you will be provided with the final registration form to complete the onboarding process.
3. Transfer of Medical Records (if applicable):
If you currently have a family physician and would like us to obtain your previous medical records, you will need to sign a medical release form. We will then request the records on your behalf. Please note that your previous doctor may charge a fee for transferring your medical records.

VISITS
Only one medical issue per visit to ensure that all patients receive timely and efficient care. if you have multiple issues, please book for another day.
PRESCRIPTION POLICY
We do not prescribe narcotics and controlled substances.
ZERO TOLERANCE
We do not tolerate any form of aggressive, abusive, or disruptive behaviour. This includes verbal or physical threats, harassment, or any inappropriate language.
NO SHOWS
There is a $50 no show fee. You must advise us 24hrs before your appointment to cancel.
